Job Title: Hospice RN Team Leader
Job Summary: The Hospice RN Team Leader oversees patient care delivery, coordinates interdisciplinary teams, and ensures compliance with care plans and standards. Manages nursing schedules, supports admissions and discharges, and acts as a liaison between patients, families, and the care team to provide high-quality, compassionate care.
Job Qualifications:
Education: Graduate from an accredited Diploma, Associate, or Baccalaureate School of Nursing.
Licensure: Current state license as a registered nurse (RN) and/or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact. Current driver's license. Valid and current CPR certification.
Experience: Minimum of two years experience as a registered nurse in a clinical care setting. Hospice experience required. Management experience required.
Skills: Nursing skills as defined as generally accepted standards of practice. In-depth knowledge of hospice regulations, Medicare/Medicaid guidelines, and best practices in end-of-life care. Strong leadership, communication, and problem-solving skills. Compassionate, empathetic, and committed to the mission of hospice care. Must read, write, and comprehend English.
Transportation: Reliable transportation and valid auto liability insurance.
Environmental and Working Conditions:
Performs duties in an office environment during operating hours. If required to make patient visits, may work in patients' homes or facility in various conditions; possible exposure to blood and bodily fluids and to infectious diseases; ability to work flexible schedule; ability to travel locally; some exposure to unpleasant weather; PRN emergency calls. Must be available to work on call.
